While some can speak English and Mandarin proficient, unfortunately , some elderly patients did not attend school due to lack of finances at home, or during Japanese occupation and so forth. The younger generations may not be able to speak the dialects of elderly patients. Also, the context may be perceive differently. Lastly, even though some elderly may not understand doctors, there is fear of clarifying. It is good to bring up awareness of language and context. Also, it takes effort to build trust. Lastly, lets keep language simple, and avoid jargons and paralanguage.

I believe that under language barrier we should not only include native tongue,but medical terminology as well. Medical terms are much like legalese and require a years of study to truly grasp. It's important to remember many of these terms are not understood by our patients. Not only are they not understood the power dynamic may make some too ashamed or embarrassed to ask for clarification.

Bring back some form of clipboard or tablet with imperative communications and have it follow the patient through hospital departments, like old charts did, obligate it being read aloud before procedures. Save lives by being analog, even for just this, and maybe for power failure/IT downtimes?
