Now, all eligible Aetna Medicare Advantage members across the state, who meet the program’s criteria, will benefit from Monogram’s value-based, in-home, multispecialty care for patients suffering from polychronic disease.    Like many states, Florida faces a shortage of healthcare professionals, rising healthcare costs, and the need to expand interventional services. Monogram Health has a proven track record for driving positive clinical outcomes, decreasing hospital and ER admission rates, and increasing star ratings, resulting in reduced medical costs and a high quality of living for all engaged patients.    To date, the CDC reports that kidney disease, heart disease, diabetes, and lower respiratory disease are among the top 10 leading causes of death in Florida.
