I recently spent time with our Oak Street Health, part of CVS Health teams in Chicago, visiting our Bronzeville and Englewood centers and connecting with colleagues at the Treehouse for our CVS Health Company Town Hall watch party. At our Englewood center, I was joined by several leaders including Anand Nagarajan, C. Thomas Xu, Marisa Rogers MD, MPH, Michele Mitchell, MD, MBA, Alex Kreig and Selena M. Thompson, MBA. It was inspiring to see our mission and care model come to life as we observed our care teams interacting with patients, delivering the personalized, value-based primary care that makes Oak Street unique. Every person’s health journey is unique, which is why we focus on providing individualized, compassionate care that puts patients and their loved ones at the center, every step of the way. A recent effort by one of our teams at Oak Street Health, part of CVS Health truly embodies our value-based care model, which prioritizes meaningful health outcomes over the number of services delivered. When the son of an Oak Street Health patient reached out during a severe health crisis at home, he was facing the unimaginable. His mother, reluctant to seek out care, ultimately went into cardiac arrest and suffered a severe brain injury. With no advance directives in place and the family unprepared to make end of life decisions, her 18-year-old son was suddenly responsible for navigating overwhelming and complex choices. Our team stepped in immediately to offer not only clinical support, but also compassion, guidance, and hands-on help. They identified a trusted family friend to assist with decision making, and through collaborative work of our transitions, utilization management, and population health teams, coordinated with the hospital to ensure a smooth discharge. The patient was placed in a long-term care facility fully equipped to manage her needs, bringing stability and relief to her family during an incredibly difficult time. This is what whole-person care looks like — not just treating conditions, but showing up for patients and families with empathy, expertise, and unwavering commitment. Thank you to our dedicated teams for exemplifying what health care should look like and making a lasting impact for the patients and families we serve every day.
