Prepare. Transform. Improve. Guided by these three words, Practice Transformation Institute (PTI) was established in 2007. As Michigan’s first IACET-accredited provider of CME and experiential learning programs, PTI focuses on primary care transformation and community-based care team initiatives. PTI embraces a forward-thinking mindset and proactively incorporated team-centered advances in the evolution of the Patient-Centered Medical Home with Community Health Worker (CHW) training and CHW partnership development. Utilizing expert curriculum, PTI offers dynamic learning whether virtual or in-person. Using proven learning methods and strategies, while integrating adult learning approaches, PTI empowers and energizes clinical and non-clinical personnel through customizable education programming. Our blogs have talked a lot about community health workers (CHWs) and their role in helping to build a healthier community. CHWs are an advocate and liaison to connect individuals and families to health care and social services. They have a strong connection to the community they serve and may share the same ethnicity, race, language, socioeconomic characteristics, cultural, spiritual beliefs, and life experiences found in the community. CHWs can work in urban or rural environments with low-income, minority, immigrant, underserved communities, and populations with limited English proficiency.
