With a focus on improving care for patients with opioid and stimulant use disorders, Trinity Health Of New England was proud to contribute to the development of a national toolkit—created in partnership with three clinical teams and supported by the Centers for Disease Control and Prevention Trinity Health Of New England was chosen for our implementation of Peer Support Specialists (PSS) who are integrated into the care team and work to interrupt the cycle of readmission. Upon discharge from treatment, patients are connected with rehabilitation centers, outpatient programs and clinics. PSS ensure that patients have appointments scheduled and ensure continuity of care to prevent lapses in treatment. As part of the care team, PSS assist in providing wraparound support for barriers like transportation, housing instability, and insurance issues, ensuring patients remain engaged in their recovery. “Internal, interdisciplinary collaboration was key to our success especially as we built a model that leverages Community Health and Well-Being’s approach to integrated peer-to-peer support within the inpatient care team,” said Carolyn Alessi, MBA, Regional Director, Community Health and Well-Being & Curtis D. Trinity Health Of New England is proud to announce Saint Francis Hospital and Saint Mary's Hospital have both received recertification as Trauma Centers by the American College of Surgeons (ACS). Saint Francis is verified as a Level I Trauma Center while Saint Mary’s is verified as a Level II Trauma Center. Standards for Trauma Center distinction denote the spectrum of care that must be available to the injured patient at the facility. “Saint Francis and Saint Mary’s receiving trauma center recertification by the American College of Surgeons means Trinity Health Of New England has the capacity and capability to provide the most advanced life-saving measures available and underscores our commitment to the communities we serve in Hartford, Waterbury, and the surrounding areas,” said Montez Carter, PharmD, FACHE, President & CEO, Trinity Health Of New England.
