Job Description
If you are looking for a collaborative, dynamic, physician-led practice environment where you can learn, grow, and excel in providing integrated, multidisciplinary, patient centered care, then Summit Health / Starling Physicians is the place to be! We are currently seeking a licensed Advanced Practice Nurse (APN) or Physician Assistant to work collaboratively with our Cardiology Physicians providing patient care in the outpatient office.
Requirements Include
Current Connecticut State APN or PA License
Board Certified
Cardiology experience preferred
BLS and ACLS certification required
Excellent clinical judgment, communication and time management skills
Comfortable working both independently and collaboratively in a fast-paced outpatient environment
Responsibilities Include
Conduct comprehensive patient assessments, including history, physical examination, and diagnostic evaluations in an outpatient setting
Manage and monitor chronic cardiac conditions such as heart failure, arrhythmias, hypertension, and coronary artery disease
Interpret ECGs, echocardiograms, stress tests, and other cardiac diagnostics
Perform stress tests
Review and manage test results, labs, and imaging; communicate findings and next steps with patients promptly
Provide patient advice and triage over the phone for individuals experiencing symptoms such as chest pain, palpitations, shortness of breath, or medication concerns
Develop, implement, and adjust treatment plans in collaboration with supervising cardiologists
Educate patients and families about heart disease prevention, medication adherence, and lifestyle modifications
Coordinate care with other specialists, primary care providers, and ancillary staff
Maintain accurate and timely electronic medical records (EMR) documentation
