Are you passionate about helping individuals experiencing homelessness secure permanent housing and supportive services? MHA is seeking a Case Manager to join our dedicated Housing Services team. In this role, you'll provide outreach, assessment, service coordination, and housing placement support to individuals transitioning out of homelessness.
Schedule: Part-time, 20 hours/week, Mon-Fri 1p-5p
Key Responsibilities:
Engage participants through outreach, enrollment, and assessment
Develop person-centered, trauma-informed action plans focused on housing stability
Connect individuals to vital services such as mental health care, substance use treatment, legal support, and job training
Provide crisis intervention and develop pre-crisis plans
Collaborate with community partners, state and local agencies
Offer transportation assistance and support participants in achieving their goals
Maintain accurate documentation and meet regulatory requirements
Requirements
6+ months of experience working with individuals experiencing homelessness, addiction, or psychiatric symptoms
Valid driver's license, reliable vehicle, and auto insurance
Ability to work independently and as part of a team
Strong interpersonal, communication, and organizational skills
Bilingual (Spanish) and/or Bi-Cultural candidates encouraged to apply
