What You’ll Do:

• - Provide skilled nursing in private homes
• - Administer medications, treatments, and injections
• - Monitor vital signs, report changes, and update care plans
• - Deliver patient education to clients and families
• - Perform advanced nursing tasks (e.g., wound care, G-tube, trach, catheter)
• - Maintain accurate and timely documentation
• - Represent Affirmed with white-glove professionalism and heart
  
  

Who We’re Looking For:

• - Active LPN license in Connecticut
• - 1+ year of experience (home care, hospice, rehab, or facility)
• - Strong clinical skills and confidence in independent practice
• - Reliable transportation and valid driver’s license
• - Professional communication and bedside manner
• - CPR/BLS certification required
• - Familiarity with electronic charting preferred
