BHN is looking for a mission-driven, community-oriented Case Manager to join the STAR-H Program, a regional initiative serving adults experiencing chronic homelessness and co-occurring mental health and substance use disorders.
What You’ll Do:
As a Case Manager in the STAR-H Program, you will provide trauma-informed, community-based wraparound services to individuals facing housing instability and co-occurring disorders. This includes delivering evidence-based interventions as part of the MISSION model—an integrated treatment approach that supports recovery, independence, and long-term housing stability.
Key Responsibilities:
Provide high-quality case management and outreach to 15–20 individuals with complex clinical needs
Use evidence-based co-occurring disorder interventions in both individual and group settings
Deliver MISSION wraparound services including peer support, vocational support, and trauma-informed care
Conduct outreach in shelters, court settings, clinics, and community spaces
Coordinate care with local partners including BHN’s Project Hope Team, the Springfield HUB Network, and the District Court Wraparound Team
Link individuals to community, state, and federal benefits (e.g., MassHealth, DMH, DPH)
Document all services accurately and timely in accordance with program and agency requirements
Engage in professional development through training, supervision, and conferences

Who You Are:
Master’s Degree in Social Work, Psychology, Human Services, or related field (required)
Experience working with individuals with co-occurring mental health and substance use disorders
Knowledge of harm reduction, Housing First, and trauma-informed care approaches
Strong interpersonal, communication, and case management skills
Ability to work independently and as part of an interdisciplinary team
Familiarity with local housing and behavioral health systems (preferred)
Reliable transportation and ability to work in the community (required)
