Optimus Healthcare is looking for a full-time experienced Adult Primary Care Clinician to join our team.
The Adult Remote Clinician (MD, Nurse Practitioner or Physician Assistant) will support the delivery of comprehensive primary care services through telehealth, with a focus on high-quality, patient-centered care. This role is responsible for conducting virtual visits, managing routine and acute clinical needs, and supporting care continuity through the Epic EHR system.
Orientation and onboarding will be onsite in Stratford/Bridgeport CT
A key component of this position includes managing and responding to clinical in-basket items on behalf of clinicians who are out on PTO, medical leave, or who have exited the organization. These responsibilities include reviewing lab and imaging results, responding to patient messages, processing refill requests, and ensuring time-sensitive issues are addressed appropriately.
The clinician will work in collaboration with physicians, care teams, and operational staff to support clinical workflows and uphold quality standards in a remote setting. This position is fully remote, within the United States, offering schedule flexibility while maintaining a strong connection to our organizational mission and commitment to accessible, equitable care. A Physician in this role may also serve as supervising physician for physician assistants or collaborating physician for nurse practitioners within the organization as needed.
Essential Functions & Responsibilities
Manage clinical in-baskets including reviewing and responding to test results, patient messages, medication refill requests and care coordination tasks.
Provide coverage for clinician in-baskets when clinician is on leave, PTO or are no longer with the organization, ensuring timely and appropriate patient follow-up and continuity of care.
Communicate with patients and care team members regarding time-sensitive issues that arise in the absence of the primary care clinician.
Provide remote patient care through telehealth (video or phone) visits for acute, chronic, and preventative health needs.
Conduct comprehensive health assessments, including review of medical history, medication reconciliation, and lifestyle evaluations.
Develop and implement treatment plans for common primary care conditions under established protocols and scope of practice.
Order, interpret, and follow up on diagnostic tests (e.g., labs, imaging) and adjust care plans accordingly.
Prescribe medications and other therapies in accordance with state licensure and organizational protocols.
Provide patient education and counseling related to chronic disease management (e.g., diabetes, hypertension), lifestyle modification, and preventative care.
Identify and manage behavioral health concerns as appropriate, collaborating with behavioral health clinicians for integrative care.
Coordinate referrals to specialists and other services as needed.
Participate in care team huddles or meetings to virtually coordinate care with physicians, nurses, care managers and other team members.
Maintain accurate, timely, and complete documentation in the electronic health record.
Adhere to clinical guidelines, quality standards, and organization policies, including HIPAA and telehealth-specific regulation.
Perform other duties as assigned.
Education
JOB QUALIFICATIONS/REQUIREMENTS
Must hold MD or DO degree from an accredited medical school and is board certified
Must be a graduate of an accredited school of nursing and successful completion of an approved Nurse Practitioner program and board certified by the American Nurses Credentialing Center (ANCC) and the American Academy of Nurse Practitioners Certification Board (AANPCB)
Graduate of AMA accredited Physician Assistant program and board certified by the National Commission on Certification of Physician Assistants (NCCPA)
EXPERIENCE: Required Experience is Minimum of 2 years providing direct patient care in a primary care setting as a member of an interdisciplinary care team.
LICENSURE / CERTIFICATION: Must be licensed to practice in the State of Connecticut.
CPR Certification Preferred But Not Required.
LANGUAGE SKILLS: Bilingual English, Spanish desirable, but not required.
