Why hallucinations happen in LLMs—it’s not simply a case of the model being “wrong.” Research from Anthropic’s team shows that hallucinations arise from an internal process breakdown between two distinct circuits: Answer Generator – Trained as a powerful next-word predictor, it produces what sounds most plausible and coherent based on patterns in the training data. Confidence Assessor – A parallel circuit acting as an internal fact-checker. It doesn’t know the answer itself but judges whether the model should know it, based on how “famous” or well-supported the information is. A hallucination occurs when the confidence assessor misfires, incorrectly signaling that the model has sufficient knowledge. This compels the generator to commit to an answer—even if it invents details to sound convincing. This finding further highlights the importance of knowing the inner workings of LLMs to enhance AI safety and accuracy. Thus far LLM development has been driven by a race to scale, apply and commercialize their capabilities. As AI continues to rapidly permeate all aspects of knowledge systems, it will become increasingly important to understand how these systems generate their responses. This will come through a combination of the work Farhad Davaripour, Ph.D. cites below, and providing facts derived from knowledge graphs to anchor the model responses in well bounded context.

The author argues that algorithms: PPO, RL, Transformers, and so forth, were the first half of the AI game, but now the direction must change. Benchmarks are slowly becoming obsolete.
