Agents Assemble for the HL7 Language First Interoperability Connectathon! My bags are packed, and I'm thrilled to head to Pittsburgh for the HL7 FHIR Connectathon this weekend! We'll be participating in the "Language First Interoperability" track, where we'll test how intelligent agents can talk to each other to solve the most complex healthcare use cases. In preparation for this event, we've been working on our application, 𝗣𝗿𝗼𝗺𝗽𝘁 𝗢𝗽𝗶𝗻𝗶𝗼𝗻. It's a healthcare platform that comes with a built-in agent, 𝗣𝗼. In the video below, I share how you can use 𝗣𝗿𝗼𝗺𝗽𝘁 𝗢𝗽𝗶𝗻𝗶𝗼𝗻 to build your own custom version of Po. Before healthcare had apps and APIs, there was HL7 v3. HL7 v3 gave healthcare its first structured language — XML messaging and the RIM model and introduced CDA documents still in use today. It reduced errors, improved consistency, and supported nationwide health reporting. Even in 2025, HL7 v3 continues to power parts of health infrastructure. eCW Integration: What Every Healthcare IT Team Should Know At first glance, integrating with eClinicalWorks (eCW) looks straightforward. It supports HL7. It offers a FHIR API. The documentation checks out. But in practice, it rarely works the same way twice. Whether you’re handling ADT feeds, lab results, or patient data, the way eCW behaves can vary significantly between sites. Here’s what we’ve seen across real-world deployments: • HL7 fields like PID.4 may hold external MRNs instead of internal IDs • ORU messages might be ignored if not tied to recent orders • Some fields go missing, others behave inconsistently • FHIR token access doesn’t always guarantee permission • Integration modes like "tight" or "standalone" change how data flows When this happens, downstream systems start behaving unpredictably: Lab results go unlinked. Registration data arrives late. Patient records get fragmented. One diagnostics vendor came to us after struggling to support more than 40 eCW sites. From LIS terms to HL7 messaging, it can feel like learning a whole new dialect. 🗣️📊 Here’s a quick guide to some common EMR + Lab terminologies you’ll often hear 👇 🔹 LIS (Laboratory Information System) – The backbone of lab operations 🧬. In Cerner, PathNet acts as the LIS, managing lab workflows from sample collection to result reporting. 🔹 Orderable – The test a clinician requests (e.g., CBC, LFT). Think of it as the “menu item” 🍽️. 🔹 Resultable / Component – The discrete values returned (e.g., Hemoglobin, WBC count). 📊 🔹 HL7 ORM (Order Message) – The instruction message 📩 that tells the lab what test to perform. 🔹 HL7 ORU (Observation Result Message) – The results message 📤 sent back to the EMR (either discrete values or PDFs). 🔹 Collection Lists / Labels – Barcode-driven workflows to ensure Right Patient, Right Sample ✅. 🔹 Specimen Source – Where the sample came from (blood, urine, CSF).
