📝 Some time ago, our article “Navigating investigator-initiated clinical trials: A call for guidelines & monitoring frameworks from an Indian context” was published in the Indian Journal of Medical Research (IJMR). 👥 Authors: Dr Gunjan Kumar, Dr Jerin Jose Cherian, Dr Roli Mathur, Dr Aparna Mukherjee Investigator-initiated (academic) clinical trials are essential for answering public health and scientific questions beyond commercial interests. However, in India, they face several challenges: • ❗ Lack of clear oversight and monitoring mechanisms • 📑 Poor documentation practices and absence of trial master file standards • ⚠️ Ambiguity in serious adverse event (SAE) reporting and compensation mechanisms • 🧑⚕️ Limited institutional ethics committee (EC) capacity for review and monitoring • 🔎 Weak quality control and assurance frameworks 💡 In this paper, we propose practical solutions such as risk-based monitoring, independent causality assessment committees, stronger institutional trial units, and quality assurance practices to make academic clinical trials more robust, ethical, and impactful.
