Central Adelaide Local Health Network launch a new education pathway for their Kidney Care Team

 

Supporting self-management, one step at a time. The team at CALHN’s Renal Clinic has launched a new digital education pathway co—designed to help patients living with Stage 5 Chronic Kidney Disease.

For many patients, this part of their kidney journey can feel overwhelming. Complex information, multiple appointments, and the looming transition to dialysis. That’s why CALHN’s Kidney Care team set out to create something better: - A structured, supportive program

- Available in digital formats

- Designed to help patients feel informed, confident, and in control.

This new pathway combines face-to-face sessions, group education, and check-ins via phone or online, empowering patients to manage their care more autonomously without feeling alone.
