World Sjogren's Day: Raising Awareness About the Link Between Dry Eye and Sjogren's Syndrome

Commemorating Dr. Henrik Sjogren's Legacy

Today, July 23rd, we observe World Sjogren's Day, commemorating nearly a century since Dr. Henrik Sjogren, an ophthalmologist, first identified the syndrome that now bears his name. Dr. Sjogren made the critical connection between dry eye, dry mouth, and joint pain, recognizing them as symptoms of a single syndrome. Sjogren's Syndrome can present as a primary condition or as secondary to other autoimmune diseases such as rheumatoid arthritis, lupus, and scleroderma.

The Unexpected Connection: Arthritis and Dry Eye

Many patients are bewildered to learn that a condition like rheumatoid arthritis, primarily known as a disease of the joints, can also cause dry eye. This connection often leads to delayed identification and treatment of the problem. In addition, the dryness develops and progresses without symptoms in many cases. By the time it becomes noticeable through symptoms or complications, critical windows for early intervention may have been missed.

The Importance of Early Dry Eye Detection

In Sjogren's Syndrome and rheumatoid arthritis, the development of dry eye is a significant concern. Consequently, regular eye screenings are essential. Hydroxychloroquine (HCQ), an oft used medication for these conditions warrants routine ocular screening. Understanding and addressing the ocular aspects of these diseases are crucial for comprehensive care and preventing serious complications.

As dry eye specialists, we are dedicated to raising public awareness about the critical need to screen for dry eye in every patient diagnosed with Sjogren's Syndrome and those with rheumatoid arthritis.

Understanding Sjogren's Syndrome as a Multi-System Condition

Sjogren's Syndrome, including rheumatoid arthritis, are multi-system conditions, meaning disease activity can manifest in various bodily systems, not just the joints. Similarly, the resolution of joint pains does not mean resolution of the disease and therefore is not a justification to self-stop the medications. On several occasions, discontinuation of medications results in morbid complications involving the joints and other organs, including the eye.

The Importance of Integrated Care

Understanding the multi-system nature of these conditions is crucial for comprehensive care, in addition to the essential ocular screening for those on Hydroxychloroquine (HCQ). On this World Sjogren's Day, let us emphasize the importance of integrated care and early diagnosis to improve the quality of life for those affected by Sjogren's Syndrome and related autoimmune disorders.

I hope I get a diagnosis soon. My eyes are getting a lot worse. I have a Pure Autonomic Failure diagnosis, but I get "flare ups" thar feel auto immune. I have all the classic symptoms.

I am glad you are championing integrated care for Sjogren's patients. Management of chronic dry eyes can have better outcomes not only with a multi system approach, but also understanding the Patients' Perspective. It is important to understand why certain treatment options are not popular with patients. I recently conducted a survey on the impact of dry eyes on members of Sjögren’s India. Patients want accessible, affordable treatment which gives them a better quality of life on a daily basis. We need to look at the impact of dry eyes on work life as also personal life. After almost a century we still do not have a satisfactory treatment for dry eyes which ails 94% of patients on a daily basis...year after year.
