Joint Action on CARdiovascular diseases and DIabetes

Welcome to the JACARDI LinkedIn page! We are a collaborative initiative of 21 European countries committed to tackling the challenge of cardiovascular disease and diabetes. Our mission: to reduce the burden of these diseases and their associated risk factors at both individual and societal levels. We're committed to promoting health systems that are sustainable and equitable for all. How do we achieve this? By promoting best practices: Identifying strategies across the patient journey in six priority areas, from health literacy to screening high-risk populations, integrated care pathways, self-management support and workforce participation. Testing innovative solutions in 142 pilots in 18 European countries to improve CVD and DM prevention, management and patient empowerment. Using a standardised 15-step methodology to facilitate analysis of success factors and contextual influences. Promoting broad communication: Sharing our experiences and results benefits other countries and stakeholders across the European Union (EU).     🌍 World Patient Safety Day 2025 – Patient safety from the start! 🧡 Every year on 17 September, #WHO calls the world to unite for safer health care. This year’s theme – “Safe care for every newborn and every child” – reminds us that children are among the most vulnerable patients, and protecting them from avoidable harm is a shared responsibility. Under the slogan “Patient safety from the start!”, WHO urges action in key areas such as safe childbirth, postnatal care, medication safety, infection prevention, and early recognition of complications. Iconic landmarks across the globe will light up orange to mark the day and reaffirm every child’s right to safe and quality care. At #JACARDI, we proudly join this mission. Several of our pilots already contribute to safer care for mothers and children:  🔹 🇫🇮 Finland – Supporting culturally diverse pregnant women through co-designed tools that improve health literacy, prevent diabetes, and ensure culturally appropriate care.  🔹 🇫🇷 France – Raising awareness among women with hypertensive disorders during pregnancy about their higher cardiovascular risk, promoting follow-up care and healthier lifestyles.  🔹 🇲🇹 Malta – Empowering pregnant women at maternity clinics to better understand diabetes and CVD risks, using health literacy surveys (HLQ) to tailor guidance and support. These initiatives strengthen health literacy, promote prevention, and reduce long-term risks, directly echoing WHO’s call to action for safer paediatric and maternal care. 🧡 On 17 September, let’s work together to light up the world in orange and commit to safer care for every child, everywhere. #WorldPatientSafetyDay #PatientSafety #HealthLiteracy #MaternalHealth #JACARDI #WHO

    💡 Interventional cardiology: a key to saving lives 💡 Non-communicable diseases (NCDs) are responsible for nearly 70% of all deaths worldwide. Among these, cardiovascular diseases (CVDs) represent one of the most significant health challenges. In 2019 alone, 17.9 million people lost their lives due to CVDs, accounting for 32% of global deaths, with heart attacks and strokes making up 85% of these fatalities. 🌐This rising burden underscores the urgent need for comprehensive health strategies and the integration of advanced cardiovascular management interventions. September 16 marks the International Day for Interventional Cardiology, celebrating advancements like coronary angioplasty, a life-saving procedure first performed by Dr. Andreas Grüntzig on this day in 1977. Today, angioplasty continues to be a procedure that has saved millions of lives and improved quality of life for countless individuals globally. In response to the official 𝐂𝐚𝐥𝐥 𝐟𝐨𝐫 𝐄𝐯𝐢𝐝𝐞𝐧𝐜𝐞, JA PreventNCD has provided 𝐫𝐞𝐜𝐨𝐦𝐦𝐞𝐧𝐝𝐚𝐭𝐢𝐨𝐧𝐬 𝐟𝐨𝐫 𝐟𝐮𝐭𝐮𝐫𝐞 𝐚𝐜𝐭𝐢𝐨𝐧 𝐛𝐚𝐬𝐞𝐝 𝐨𝐧 𝐞𝐯𝐢𝐝𝐞𝐧𝐜𝐞 𝐚𝐧𝐝 𝐞𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞𝐬 𝐟𝐫𝐨𝐦 𝐨𝐮𝐫 𝐂𝐨𝐧𝐬𝐨𝐫𝐭𝐢𝐮𝐦 𝐦𝐞𝐦𝐛𝐞𝐫𝐬, representing 25 countries and 100+ organizations across Europe.     🩺🫀 Last of the NTproBNP Sonja Moore and I were delighted to present our poster on our STOP-CVD pilot within JACARDI at this year’s Health Service Executive Integrated Care Conference. Having the opportunity to share the important work of JACARDI with our national colleagues was a real highlight of our conference season. We were able to show how NT-proBNP screening can support earlier detection, better prevention, and more integrated cardiovascular care. Thank you to the organisers for creating such a fantastic space to connect, learn, and exchange ideas across the health system!
