Responsibilities:
Deliver outpatient primary care, gynecological, or psychological medication management services to women of all ages
Conduct thorough patient assessments, including history, physical exams, and diagnostic workups
Develop and manage personalized treatment plans, including prescribing medications and coordinating follow-up care
Collaborate with LifePath Experts and behavioral health staff to provide emotionally attuned, integrative care
Maintain accurate, confidential documentation in compliance with all regulatory standards
Participate in interdisciplinary team meetings and case reviews
Qualifications:
Licensed APRN, eligible to practice independently in the state of Connecticut
Experience in women’s health, primary care, urgent care, family medicine, or psychiatric medication management.
Passion for working in a collaborative, interdisciplinary, patient-centered environment
Commitment to trauma-informed care and delivering highly personalized, holistic wellness support
