HealthCare Support is actively seeking a Care Transitions Nurse Manager - RN to fill an opening with a prestigious Healthcare companyin The Bronx, NY
Daily Responsibilities for Care Transitions Nurse Manager - RN :
Will make field visits but will not carry their own caseload.
Assisting and overseeing some of the more complex cases - acting as a SME for Community Health Nurses in the field
Communicate with interdisciplinary teams, social workers, and dietitians, helping coordinate that care
Monday – Friday 8:30 am – 5:30 pm I On-Call Rotation
This is a field position*
Required Qualifications for Care Transitions Nurse Manager - RN :
Active NY Registered Nursing license
3+ years' experience as a Registered Nurse / RN
2+ years of recent experience at a Certified home health agency (CHHA)
1+ year of recent supervisory experience
