The Community Health Worker is a direct care staff who has the primary responsibility for ensuring top quality
outreach and engagement of individuals into screening, care management and navigation. This entails
ensuring the client receives quality services that meets the SCN scope of work. The CHW ensures data
collection is accurate and entered timely, weekly productivity reporting & outreach planning and participate in supervision.
Essential Duties And Responsibilities
Conduct weekly outreach, reach screening benchmarks and build a care coordination caseload at the maximum capacity of 65 with short term/long term capacity caseload.
Carries a caseload and practices client-centered and collaborative care management and service navigation with clients.
Ability to carry a caseload of up to 65 to assist in ongoing care coordination.
Ensures navigation coordination according to regulatory guidelines and standards.
Advocates on behalf of client with other services providers and in a way that takes into account health disparities and systemic racism.
Interprets and explains agency policies and procedures to clients when needed
Travels off-site outreach, office visits or home visits, meetings and trainings as needed.
Processes home remediation requests for clients who are in need of service and meet criteria
Responsible for incident reporting, writing daily logs and weekly reports on productivity and data entry
Strong attention to detail, ability to manage time effectively, organized and flexible
Self-starter, community-minded with strong relationships or people skills.
Excellent prioritization, multi-tasking, and result-driven
Ability to think creatively and has the ability to both verbally and in writing.
Ability to demonstrate a strong commitment for community outreach.
Participates in regular staff meetings and supervisions.
Must be comfortable reaching targets and goals with minimum supervision.
Performs other related duties as assigned.

EDUCATION
Minimum qualification -an associate degree in human service Field, Social Work, Community Health or Health related field strongly recommended.
At least three years of direct experience in care coordination, service navigation, case management or case assistance of vulnerable populations.
Administrative experience in human service or related program(s), including experience in quality assurance/improvement and program management.
Bilingual skills preferred (Spanish/English). Excellent communication skills and proficiency in both languages.
