Background Poor communication in the healthcare industry can be fatal Illegible prescriptions/instructions cause 26% of all hospital errors Lack of organization of data Limited assurance of confidentiality, integrity and availability of data No electronic archiving or logging
Solution - Clinical Xpert™ Navigator  Robust software with a user-friendly front end for computers and PDAs Centralization of all patient data and directives Personal devices for residents and doctors assure that the data is always at their disposal
Advantages Improves communication, and coordination of patient care Errors due to illegible instructions are eliminated The convenience of mobility Accessibility – to have all data at the residents’ disposal at all times Wireless syncing for updates
Handoff A module of the Clinical Xpert™ Navigator software  Streamlines the process of resident shift changes Residents are part of a team that provide continuous care for a single patient Updated To-Do list and Problem list shared by all residents in a team Resident of the new shift is briefed easily with minimal room for error
