This patient was admitted to the hospital with fever, chills, breathlessness, cough, body pains and abdominal discomfort. Examination found high blood pressure and reduced breath sounds. The patient has a history of diabetes, high blood tension, coronary artery disease and bronchial asthma. Tests showed increased white blood cells, blood sugar, blood urea and creatinine. Sputum gram stain found polymorphs and gram positive cocci. The diagnosis was type 2 diabetes, high blood tension, coronary artery disease and bronchial asthma. Treatment included antibiotics, antacids, diabetes and asthma medications.
