This document provides a 5-step approach to interpreting arterial blood gases (ABGs) and resolving acid-base imbalances:
1) Identify if the primary disorder is acidemia or alkalemia based on the pH.
2) Determine if the primary disorder is respiratory or metabolic based on pCO2 and HCO3 levels.
3) Assess the degree of compensation by the other system using formulas for different acid-base disturbances.
4) Calculate the anion gap to identify anion gap metabolic acidosis.
5) Use the "delta gap" to detect combined acid-base disorders or underlying alkalosis.
