The Ariane 5 rocket, launched by the European Space Agency on June 4, 1996, failed catastrophically just 40 seconds into its maiden flight due to a software error in the inertial reference system. The failure, resulting from a conversion issue with a floating point number, led to the loss of the rocket and its $500 million cargo, following a decade of development that cost $7 billion. Airbus Defence and Space was the prime contractor for the project, which is part of the Ariane program operated by Arianespace.
