- 1. Associating your notes to VC3 problems You have been asked to reach mastery in evaluating 25 core problems across your clinical experiences. KM Learning Portfolio captures all of your notes in StarPanel. This provides an excellent means to track exposure to each presenting problem. This module instructs you in how to associate your notes to VC3 presenting problems.
- 2. Three ways to associate your note There are three mechanisms for associating your notes to VC3 problems. First, the system automatically grades and associates relevant notes written using StarNotes; the system does not automatically grade notes written using StarForms. Notes graded if use StarNotes. Notes not graded if use StarForms. FAQ: What clerkships routinely use StarForms? Psych and Ob/Gyn Any exceptions? The system grades some StarForms used in Internal Medicine. What does all this mean? If you use a StarForm template, you will have to associate that note to VC3 yourself and not rely on the system to do it for you. If you use a StarNote template, you’ll receive an email once a week re notes graded to be relevant by the system.

You see your patient, the note, whether it was recorded as a case of abdominal pain, the concepts of abdominal pain found in the notes, the hits, the score (google-type scoring system we created), and the sections of the note in which the concepts are found. If you confirm that this case was concerned with important concepts of abdominal pain, you may click record and this note will be added to list of notes recorded for abdominal pain.

- 14. Last question Last question: should I associate all my notes I write? Though not a hard-fast rule, for each patient to whom you are assigned and asked to write notes, you should consider associating your H&P and one progress note to VC3 topics that relate. If something changes in the patient’s course, like developing a UTI (dysuria VC3 problem), then you’d add that day’s progress note to dysuria on your dashboard if you discussed the UTI in that note. We ask that you do not associate notes to VC3 that are not written in StarPanel (eg. VA notes, notes from clinics in the community). Such notes require significant de-identification to be HIPPA compliant. Shade Tree notes are fine because they are written in StarPanel.
