Beyond just record sharing, there is a need for clinical decision support and navigation tools to help providers make more informed decisions. The document discusses implementing a single platform that would allow any provider across acute, community, social care, mental health, third sector, and primary care to share patient records and referrals in real-time. This mature, multi-agency system aims to go live supporting 650,000 patients with electronic decision support, navigation aids, and the ability to handle 0.6-0.8 electronic referrals per patient per year by December 2018.
