#22:The results from the Cox proportional hazards models used to examine AREDS data show little evidence of a detrimental effect of cataract surgery on progression to advanced AMD. Use of the Cox regression model allows us to take advantage of a unique feature of AREDS, the ability to examine the effect of important covariates, including time-dependent covariates, on progression to late AMD. Results from the Cox models are not statistically significant and there are no consistent trends among models, with point estimates for risk of neovascular AMD in the harmful direction for the matched pair analyses and in the protective direction for the logistic regression analyses. For GA results, all models had point estimates in the direction of protection, suggesting that it is unlikely that there is any risk of cataract extraction on the development of GA. The absence of any consistent pattern in the direction of harm across models reinforces the conclusion that AREDS data provide little evidence that cataract surgery increases the risk of progression to late AMD.
