Legislation Overview
The Caregiver Advise, Record and Enable Act
ensures hospital patients can designate a family caregiver
and requires hospitals to offer that caregiver instruction
and demonstrations of medical tasks they will be expected
to provide for their loved ones at home, such as
administering multiple medications, dressing wounds, and
operating medical equipment.

CAREActOverview
Main Tenets of the Legislation
 The opportunity to designate a caregiver
• “A hospital shall provide each patient…with at least one opportunity
to designate at least one caregiver…”
 The notification of movement to the caregiver
• “A hospital shall notify the patient’s designated caregiver of the
patient’s discharge or transfer to another hospital or facility…”
 The instruction of a caregiver in all after-care tasks
• “Prior to a patient’s discharge…the hospital shall consult with the
designated caregiver regarding the caregiver’s capabilities and
limitations and issue a discharge plan that describes a patient’s after-
care needs at his or her residence.”

The Problem:
Carla is the caregiver for
her father Andrew who
has diabetes. Andrew
recently was released
from the hospital after a
bad fall. He came home
with new medications
and needs to have his
wounds taken care of
2x/day. Carla has no
medical training and is
unsure of what to do.
Our Solution:
As her father’s designated caregiver, Carla is notified
by the hospital that he is being discharged. She
speaks with the nurses about her father’s care plan
and signs up to receive care messages which will
include medication, prescription refill, and follow up
appointment reminders, instructional videos with detail
on how to properly clean his wound and change the
dressing, potential infection signs to look out for, and
secure chat access to medical professionals who can
answer her questions in real-time to ensure Andrew
stays healthy and Carla knows how to provide him
with the best care.

Carla was given a stack of
paperwork from the nurses
detailing how to help manage
her father’s condition,
however she misplaced it.
Good thing she signed up to
receive condition information
and instructions right to her
phone, complete with videos
that she can bookmark for
access at anytime, anywhere.
GoMoHealthCanHelp
Carla is getting ready for the day.
Just then she gets a text from the
hospital with a reminder to make
sure Andrew takes his medication.
She would have forgotten
otherwise. She gets another
reminder that night.

Thursday has been rough for
Carla. Working a full-time job
and taking care of her father
is stressful. Her phone beeps,
it’s a motivational text
message with a story about
how another caregiver in a
similar situation is handling it.

On Friday, Carla gets a text
with a link to a survey about
how her father’s wounds are
healing. She notes that she
is worried about the bruising
around the wound. A nurse
then contacts her through
secure chat, sending her a
link to a video on bruising
and answering all her
questions in real-time.

  • 31. The Problem: Carla is the caregiver for her father Andrew who has diabetes. Andrew recently was released from the hospital after a bad fall. He came home with new medications and needs to have his wounds taken care of 2x/day. Carla has no medical training and is unsure of what to do. Our Solution: As her father’s designated caregiver, Carla is notified by the hospital that he is being discharged. She speaks with the nurses about her father’s care plan and signs up to receive care messages which will include medication, prescription refill, and follow up appointment reminders, instructional videos with detail on how to properly clean his wound and change the dressing, potential infection signs to look out for, and secure chat access to medical professionals who can answer her questions in real-time to ensure Andrew stays healthy and Carla knows how to provide him with the best care. © 2015 Gold Group Enterprises, Inc.
