Program Planning Project HighlightsNicole Small
Review the results of the Community needs assessment to begin to develop intervention strategies
Create a health outcome reflecting improved knowledge of a healthy diet to reduce heart diseaseReducing the burden of disease of obesity and heart disease by addressing the risk factor of improper diet. Based on the Community Needs Assessment the priorities of the program are to:
Define Program Goals and Objectives Increase basic knowledge of healthy lifestyle choices, focusing on healthy eating for the heart and weight maintenance for worksite employees.Creating a supportive environment for lifestyle change for worksite employees by providing access to knowledgeable professionals.
Proposed Intervention: Education table located centrally in the worksite that will provide written information on healthy eating, weight maintenance, smart grocery shopping for heart health. Dietitian and nurse educators will be available to answer questions and provide information to worksite employees and other individuals in the area (patients/families of patients).
