Context Focused Therapy
Tehran CP Workshop, May 2017 2
Child Focused Therapy vs Context
Focused Therapy
 In the past, treatments for children with CP have focused
on changing the child’s function by trying to change their
abilities like strength, movement and coordination. This is
known as Child Focused therapy.
 A different approach is to focus on changing the activity or
the environment to allow children to participate in their
desired activities in whatever ways they can. This is known
as Context Focused therapy.
Tehran CP Workshop, May 2017 3
Context Therapy Theories
 Dynamic Systems: Success at functional goal depends on
interaction of factors within child, task and environment
subsystems
 Family centered: Collaborative relationship between
family and health-care providers to identify goals and
intervention strategies
Tehran CP Workshop, May 2017 4
Dynamic Systems Theory
 Dynamic systems theory
posits that motor behaviors
are organized around
functional tasks or goals
and that the specific motor
solution is influenced by
the spontaneous
interaction of variables
from three sources: child
characteristics, task
demands, and
environmental influences.
Tehran CP Workshop, May 2017 5
Child characteristics
 Child characteristics represent not only the traditional
physical impairments considered in the rehabilitation of
children with CP (e.g. muscle tone, range of motion,
balance) but also non-physical characteristics such as
motivation, attention, and cognition.
Tehran CP Workshop, May 2017 6
Task Demands
Examples of task characteristics (what the child is trying to
do) are:
 shape, size, and weight of a ball for the task of throwing
 size, shape, or texture of a writing instrument for the task
of printing
 consideration of alternative solutions such as a computer
or label maker.
Tehran CP Workshop, May 2017 7
Environmental Influences
 Environmental considerations are the identification of
physical, social, and attitudinal influences that
represent barriers to or facilitators of successful
completion of the identified task or goal.
 Examples include physical accessibility, availability of
assistants, and the attitudes of persons within the
child’s environment.
Tehran CP Workshop, May 2017 8
Tehran CP Workshop, May 2017 9
Context Focused Therapy Effectiveness
 Darrah et al (2011)proposed a new approach named CFT
for children with CP
 Law et al (2011) showed that child- or context-focused
therapy approaches are equally effective
 Novak (2013) showed context therapy as a good
intervention.
Tehran CP Workshop, May 2017 10
Cerebral Palsy and Sports
 Children with CP have as much of a
desire as any other child to take part
in sports, recreation, and play, but
sometimes, they cannot participate
in a traditional way.
 For every form of play and
recreation, there is a modified
version that makes participation a
possibility.
 People with CP can participate in
Paralympics in different courses such
as football 7-a-side.
Tehran CP Workshop, May 2017
11
Cerebral Palsy and Sport
 Sherrill has been involved in the
development and promotion of sport
for individuals with CP
 There are eight classifications in
(United States Cerebral Palsy Athletic
Association ) sports:
 Classes 1-4 are non-ambulatory
 Classes 5-8 are ambulatory.
 The classifications are broken down by
specific level of function and have
some potential implications for
developing exercise guidelines for
persons with CP.Tehran CP Workshop, May 2017 12
Importance of sport
 The importance of sport for
persons with CP was
emphasized from both a
physical and psychological
standpoint.
 sport is an excellent
medium for changing
society's attitudes towards
persons with CP.
Tehran CP Workshop, May 2017 13
Assistive Technology(AT)
 AT Device: Any item, piece of equipment
or product system whether acquired
commercially off-the-shelf, modified, or
customized that is used to increase or
improve functional capabilities of
individuals with disabilities.
 AT Service: Any service that directly
assists an individual with a disability in
the selection, acquisition, or use of an
assistive technology device.
Tehran CP Workshop, May 2017 14
Terminology/Topics to Consider
 Least restrictive environment
 No prerequisites to try AT
 Universal Design
Tehran CP Workshop, May 2017 15
Categories of AT
 Augmentative and Alternative
communication
 Activities of Daily Living
 Seating, Mobility and
Positioning
 Adapted computer keyboards ,
Adapted Mouse , Head based
controls, Word prediction and
talking word processors
 Environmental control/EADLs:
Switch access for cause and
effect, toys, leisure
Tehran CP Workshop, May 2017 16
Cerebral Palsy and AT
 Varies with type of CP:
spastic, athetoid, etc.
 Athetoid connect most
with AT
 AT to support overall
independence,
community skills,
communication, work,
leisure
