                A renewal guarantee for group health

During a challenging economy, the ability to set a health plan budget for multiple years is a
critical advantage.

Under development for five years, this combination of risk management services has been
so successful in controlling healthcare costs that it is now offered with a financial guarantee
at inception and renewal.

COVENANT ADVANTAGE Highlights

• Available to employer groups with 200+ participants.
• Provides an annual cap for renewal costs related to trend and claims.
• Employer agrees to implement three services with our approved vendors:

   1. Adopt our wellness model connecting plan design to biometric test results.
   2. Establish a primary care on-site clinic.
   3. Install “MAP” (Medical Advocate Program), a special navigator service that
      supports members facing complex medical decisions.

The Guarantee

    First year plan costs are quoted along with a guaranteed maximum renewal based
     on compliance with the above mentioned programs.
    At first year renewal a guaranteed maximum will be offered for the third year. At
     second year renewal a maximum is offered for the fourth year, and so on.
