Learning Outcomes By the end of this presentation you should be able to : describe evidence-based practice explain why it is important and outline the five steps of EBP
Definition “ the integration of best research evidence with clinical expertise and patient values” Sackett DL, Rosenberg WMC, Gray JAM, Haynes RB, Richardson WS. Evidence based medicine: what it is and what it isn’t. BMJ, 1996;312:71-2
What is EBP? Evidence based practice is the integration of: Best evidence clinically relevant, methodologically sound Clinical expertise education, experience, clinical skills Patient values Preferences, concerns, expectations
Why is EBP important? Provide quality patient care Meet specific information needs Select clinically relevant and applicable research Reduce implementation delays Keep knowledge correct and up-to-date Satisfy purchasers
How do we do EBP? Formulate an answerable question Finding the best available evidence Appraise the evidence Apply the evidence in combination with clinical expertise and patient values Evaluate outcome
Formulate an answerable question Triggered by patient encounters Relevant to your practice A well built question makes the search for evidence easier
Find the best available evidence Identify search terms Select a resource Formulate a strategy Search each important concept separately Combine the separate sets of articles Limit the results
Appraise the evidence Appraise for Validity and Applicability Use a simple critical appraisal method that will answer these questions: What question did the study address? Were the methods valid? What are the results? How do the results apply to your practice?
Apply the Evidence Integrate the evidence with clinical expertise and patient values Incorporate this clinical decision into practice justified as evidence based
Evaluate the Outcome Was the application effective? Should it continue to be included in practice? How can my clinical decision making process be improved?
Keep it simple Keep a log of questions Answer a few a week Get help with searches Simplify appraisals Focus on interpreting the evidence
Summary Simple 5 step process Patient focused questions Plan searches Appraise evidence Apply Evaluate outcome Continuous patient care and clinical practice improvement
The Big Picture Building a question PICO questions Type of question Type of study Search strategies MESH terms Combing results Limiting results Critical appraisal Validity applicability
