This document summarizes an unusual case of a 54-year-old man who developed Escherichia coli (E. coli) meningitis without any history of trauma. He was brought to the emergency room after having a grand mal seizure. Despite being treated with antibiotics, his condition did not initially improve. However, against expectations, after 6 days in a coma he regained consciousness and made a full recovery, which is highly unusual for a case of spontaneous E. coli meningitis in an otherwise healthy adult. The case highlights the extreme rarity of developing this type of meningitis without any known portal of entry for the bacteria.
