This document outlines steps for conducting a head-to-toe physical assessment. It includes assessing the head for cerebrospinal fluid leaks and broken teeth, pupils, breath odor, and neck for jugular vein distention, tracheal deviation, and medical alert tags. The chest, abdomen for distention or evisceration, pelvis, and all extremities should be examined. Capillary refill time and additional medical alert tags should also be checked, along with assessing the back.
