CQI – Continuous Quality Improvement
Audits
There were monthly/bi-weekly audits conducted at each facility. When I
came on the audits were done as questions with yes no answers and the
process was to manually count and summarize at the regional office.
One of the first projects I undertook was to automate this process.

 I automated the process, instead of manually counting the yes and no, a “1” was
placed under yes, no, or n/a which would automatically count.

 The cover page would automatically tally and graph the results, which would result
in real time the site being able to see the results and not have to wait for the
summary from the regional office for their results.

Audits - Region Summary
For summarizing the statewide results I wanted to create an easy process that would
automatically update and populate results and require very little time for regular
maintenance. Here I wish to demonstrate my knowledge of automating and linking cells.

1. The health chart is labeled with the Patient's Name, DOC/SID#, and Volumes?

7 Day Intake Physical Exams
Every inmate admitted to an intake facility required an intake physical within 7 days.
Here I would like to present how I summarized this report to show my knowledge of
IF STATEMENTS and VLOOKUP formulas.
(Above) This represents a listing of inmates that I would receive from the State
of Maryland on a daily basis in which we were required to show on a monthly
report if they received a 7 day intake and when. Monthly numbers could
range between 1,000 – 2,000 inmates.
I would run a system report (not shown, that would be on a separate tab) that would be matched up
against the list of inmates that require a 7 day intake. Above is the template I created that would
automatically do this analysis. In column D, I would copy and paste the data received from the State of
the required inmates that need a 7 day intake. The other cells would do a match with the ID number
against the system report and tell me various pieces of information that I wanted to know.
Column C – shows which facility the intake was performed at, or that one was not done.
Column C formula:

Column P & R – shows the date the intake was completed one in descending order by date, the other by ascending order by date.

Column U - would look at column P and R and select the cell with the lower days variance.

7 Day Intake Physical Exams
For the final report of the 7 Day intakes I used a drop box with an automatic color code of
the row using an INDIRECT formula.
In the conditional formula setting under the rule using a formula, the formula is as
follows=INDIRECT("L"&ROW())="3 Completed but greater than 7 days on NextGen"
The summary tab
would automatically
count and summarize
all the information on
the spreadsheet and
break it down to
percentages using
countif formulas.

ARPS and Grievances
As I started to automate my workload I started to have “free” time on my hands. With this
time I started to work with other departments and help them to standardize their formats
and analyzing protocols. Using similar formulas as listed above I automated the graphing
and reporting for ARPS and Grievances giving high level breakdowns.

Prior to my working with this department, the only trend was done on grand totals.
After standardizing the spreadsheets using drop boxes I then used array formulas and
vlookups to automatically populate high detail graphs showing full detail for every
item reported, broken down by Statewide, by Region, and by Site, which I made
into an automatic update of the graphs once the raw data was dropped in.

Overtime Analysis
I continued working with the other departments improving processes in place. I then
started to draw the attention of the Vice President of Operations. At that point I started
doing special projects for her, which lead to being sent to the corporate office to be trained
in SQL and start working on financial data trends which were previously only done from the
corporate office. The queries ran from SQL consisted of over 100,000 rows when dropped
into excel. I would then create a pivot chart and that would automate totals and then utilize
dynamic graphs. A dynamic graph is a special type of graph that will automatically populate
each month dropping the month prior. Here I wish to demonstrate my knowledge of being
able to roll up for summary large amounts of data in a very clean and precise way.

Erik Laskowitz has been a business associate for approximately two years. During this time Erik has
made himself indispensable with his efforts to assist myself and other staff with developing charts
