This document contains a job performance evaluation form for rating an employee's (referred to as "is manager") performance. It includes sections for reviewing performance factors, employee strengths and areas for improvement, developing a performance improvement plan, and obtaining signatures to finalize the evaluation. Guidelines are provided for rating performance on a scale from "Outstanding" to "Unsatisfactory". Examples of performance review phrases are also given for evaluating different aspects of performance such as attitude, creativity, decision-making, and others. The form appears to be a comprehensive tool for formally assessing an employee's work.
