Fragmentation – Data Everywhere
• Personal Health Records (PHRs) e.g. Microsoft HealthVault,
NoMoreClipBoard, Lucy, PHIM, Humetrix
• Patient/Member Portals attached to EHRs e.g. MyChart (Epic), athenahealth,
FollowMyHealth (Allscripts), myHealthButton/Portal (MI Medicaid)
• Specialized Registries for Consents such as:
• Advance Directives (e.g. living wills, do not resuscitate, etc.)
• Organ Donor
• Behavioral health and SUD types of information (e.g. standard consents)
• Clinical trials
• Active Care Relationships (and ability to dispute provider attributions)
• Language of preference
• Literacy issue – language at 6th grade level
• Patient Navigator

Problem Statement
• National push for consumers to manage their own health information
• Consumers need to control access to their data
• Start with Medicaid beneficiaries
• No consistent electronic place today where consumers can specify
preferences regarding how and where to share patient-controlled data
• Providers have no central service to retrieve consumer preferences
• Fragmentation will increase without a unifying service
