3. Nursing Process
“Nursing process is a systematic rational method of
planning and providing individualized nursing care.”
• Its goal is to identify the client’s health care status, potential
and actual health problems, to establish plans to meet the
identified needs and deliver specific nursing interventions to
address these needs.
4. Definition
“An organized sequence of problem-solving steps used to
identify and to manage the health problems of clients.”
It is accepted for clinical practice established by the
American Nurses Association.
5. Purposes of Nursing process
Purposes of nursing process are:
• To identify the client’s health status.
• To identify actual or potential health care problems.
• To establish plans to meet the identified needs.
• To deliver specific nursing interventions to meet these needs.
• To determine whether to continue, modify, or terminate the plan of care.
• Purpose is to provide client care that is :

• Holistic
• Effective

Assessment involve:
• Collection of data
• Organizing the data
• Validating the data
• Documenting the data
Assessment is the systematic and continuous collection, organization,
validation, and documentation of data (information).
13. Implementation
Implementing normally includes the following steps.
• Reassessing the client
• Determining the nurse’s need for assistance
• Implementing the nursing interventions
• Supervising the delegated care
• Documenting nursing activities.

“Evaluation is a planned, ongoing, purposeful
activity in which the nurse determines.’’
(A) the client’s progress toward achievement of goals/outcomes and
(B) the effectiveness of the nursing care plan.

Evaluation include the following steps.
• Collect the data related to outcomes.
• Compare data with outcomes.
• Relate nursing actions to client’s goal.
• Draw conclusion about problem status.
• Continue, modify or terminate the care plan.

• Gordon’s Functional Health Patterns is a method develops By
Marjorie Gordon in 1987 proposed functional health patterns as a
guide for establishing a comprehensive nursing data base.
• Gordon's functional health patterns:
Gordon's functional health patterns is a method used
by nurses in the nursing process to provide a more
comprehensive nursing assessment of the patient.

By using these categories it’s possible to create a systematic and
standardized approach to data collection, and enable the nurse to
determine the following aspects of health and human function.
1. Health perception and management
pattern:
It focuses on the person’s perceived level of health and
well-being, and on practices for maintaining health. Also evaluate
Habits including smoking and alcohol or drug use.
20. 2. Nutrition and Metabolic Pattern
• It focuses on the pattern of food and fluid consumption relative to
metabolic needs.
• Actual or potential problems related to fluid balance, tissue integrity,
may be identified as well as problems with the gastrointestinal system.
21. 3.Elimination- Excretion Pattern
• It focuses on excretory patterns (bowel, bladder, constipation,
incontinence, diarrhea.
4. Activity- Exercise Pattern.
It’s focused on the activities of daily living requiring energy
expenditure, including self-care activities, exercise, and leisure
activities.
22. 5. Sleep- Rest Pattern
• It focuses on the person’s sleep, rest, and relaxation practices.
• To identified dysfunctional sleep patterns, fatigue, and responses to
sleep deprivation.
6. Cognitive- Perceptual Pattern
• It’s focused on the ability to comprehend and use information and on
the sensory functions.
• Neurologic functions, Sensory experiences such as pain and altered
sensory input.

Its focused on the person’s attitudes toward self, including identity,
body image, and sense of self-worth.
8. Role- Relationship Pattern.
It’s focused on the person’s roles in the world and relationships with
others.
Evaluate Satisfaction with roles, role strain, or dysfunctional
relationships.
24. 9. Sexuality-Reproduction Pattern
• It’s focused on the person’s satisfaction or dissatisfaction with
sexuality patterns and reproductive functions.
10. Coping-Stress Pattern.
It focuses on the person’s perception of stress and coping strategies
Support systems, evaluated symptoms of stress, effectiveness of a
person’s coping strategies.
