This document discusses outfitting-related structural defects found on double-hull tankers. It notes that about 5 out of 6 defects in the upper deck plate and stiffeners were due to cracks caused by outfitting details being neglected. It also recommends outfitting inspections focus on compliance with outfitting details and be conducted at the block stage when possible. Typical design considerations are outlined such as using open support sections, avoiding pipe sections, ensuring proper backing structure and transitions, and limiting bracket toe heights. Examples of defects like fractures at bracket toes and cracks from lack of backing structure are also provided.
