PercutaneousTranshepaticCholangiographyPTC is a preoperative radiologic contrast study with a focus on the bile ducts of the liver, gallbladder, and surrounding tissues.
What is a PTC?A needle is inserted through and across the liver and into the bile ducts, with the aid of fluoroscopy. Contrast media is injected and the biliary ducts are examined for abnormalities or blockages.Bile may be withdrawn for analysis. Spot x-rays may then be taken.
Why would someone need a PTC?For patients with obstructive jaundice Which when the patient is yellow and the ducts are suspected to be dilated or obstructed due to calculi or biliary stenosis.Stone extraction and biliary drainage: The radiologist can remove a stone or dilate a restricted duct so that bile can be drained. When CT shows an obstruction, but the cause of obstruction is unknown.
What does a patient have to do before a PTC?NPO after midnight before the examAllergies to anesthetics, iodine, or shellfish. The doctor should be informed if the patient has diabetesPTC should not be performed on anyone who has colitis, massive ascites, or serious bleeding disorders
What supplies are needed for a PTC?General fluoroscopic equipment with spot film device or high quality image grabGeneral sterile procedures packSkin preparationSterile towelsLocal anesthetic equipmentChiba needleLow osmolar contrast
Routine radiographic projections for PTC?Films 14x17Center films to right upper quadrant before procedureScoutSupine – PA Oblique, right LateralErect – PA, PA Oblique, right lateralAny other images as required by radiologist
