Procedure for Closing Cases
Requirements of the Child and Family Development Worker
RAI/IS Data Base
Step 1: Upload the Final Case Plan to the RAI – IS System

Ensure all of the case notes are up to date.
Step 2: Proceed to follow remaining closure steps on the
Client Closure Checklist
Client File
Complete Client Closure Checklist in particular the following:
Form located at - S:CLIENT FILESCLIENT FILE FORMSCLIENT CLOSURE FORMS CHECKLIST
Client Closure forms Checklist
Client Name: {Enter Name} Id No: {Enter Client ID #}
Client Closure forms Checklist completed YES/NO DATE
Working Alone Assessment Form
3 Month Review Form
Client Contact Sheet
Closed on RAI Database
Case Notes Up to Date
RAI Case Closed form-New
Closed Case form to DCH’s
Transferred Client folder from Open Client folder to Clients Closed Files
Case Manager: {Enter Name}
Step 3:
 Service Coordinator is to receive the completed file within two weeks of data
base closure date.
 File to be placed in Service Coordinators “Case Closed Tray”.
Referral for Active Intervention – RAI Inala/Goodna 2
 ALL Case Notes to be up to date prior to closing.
 Three Month Review form to be completed, signed by Family, Case Worker,
Service Coordinator/Senior Practitioner and or Manager
 Risk Assessments? MUST be signed by Senior Practitioner.
All of these forms need to be completed and signed prior to closing case.

Requirements of the Service Coordinator
Shared Drive
Please ensure the Child & Family Development Worker has completed all database
requirements and have closed the case on the RAI/IS Data base first.
Follow up by closing the case manually in the shared drive.
Step1:
 Double click into “S” drive.
 Scroll down and double click into “CLIENT” file.
 Double click on “CASE ALLOCATION”.
 On the bottom of the sheet there are several coloured tabs.
 Select the Yellow coloured tab titled “REFERRALS”.
 Search for the family that is being closed.
 Fill in the “CLOSURE DATE”.
 Highlight and colour the family according to reason of case closure;
Highlighted Green Not closed - Child Safety
Red Re-notification
Purple No Engagement
Aqua Disengagement
Yellow Moved out of Area
Orange Completed
Grey Missing Information
Black Referral Not Accepted
 Save the work sheet after you have completed.
Step 2:
 Go back to the bottom of the work sheet to the coloured tabs and select
 GOODNA
 FOREST LAKE
 INALA
 OTHER (Health, Education or Other Services)
 Click on the TAB which your client case referral came from.
 Within the selected tab, scroll to look for your family.
Ensure that all the clients details are there including their “DATABASE”
numbers and “DATE CASE CLOSED”.
 Please bold and write your database number in “RED”.
 Highlight and colour columns “A-F” in green.
 Click Save.
Step3:

 Go back to the bottom of the work sheet to the coloured tabs and scroll
“LEFT” to the “CLOSED CASE STATS” coloured tab and select.
 Scroll down to the Office that your client referral came from.
 It will also have a month and year written on top of the office.
Example JAN-09 INALA.
 If the month of your case closure is not visible in the spreadsheet and you are
unfamiliar with spreadsheet format seek assistance from supervisor or team
leader prior to proceeding
 Fill out all required information and save.
Step 4:
 When all of the steps above have been completed, scan and email a copy of
the RAI Case Closed Form to the relevant department that referral was first
received.
 Save a copy of the email into the Shared Drive under the client’s file that has
been closed.
