FILLING IN THE RETRIEVAL RECORD (CASE SHEET) To be completed whenever there is any patient contact One Patient (most of time) = Expectation of Faultless Documentation Retrieval Records are in Blue Folders in ALL vehicles Need to be filled out DURING the mission once patient stable Timings are very important - try to jot down during mission on a small pad A copy of the form MUST be left with the patient at the receiving hospital If unable for any reason then a copy MUST be faxed to the hospital later

Date and Timings 1st Contact = time called Despatch = time told to go (handover completed) Depart Base = Time wheels rolling or skids off At Patient = time you contact patient (NOT helo lands) Depart Site = Time start moving from scene/hospital with patient Arrive Dest= Time you reach bed space in ICU or ED Back at Base = Time complete and ready for next job
