- 3. Presenting with the complaint of: • Altered mood • Disturted sleep • Irrelevant talk • Self neglect • Aimless wandering From 1 year ,aggravated in past 14 days
- 4. History of present illness : • My patient is under psychiatric care since 2017 /2018 and on anti pychotics prescribed from civil setup hospital swaabi . • According to secondary data ,from his brother in law he developed these symptoms after his mother's death.
- 5. Past medical history: • No history of any fever ,fits , epilepsy. • No history of IHD and DSH. • No history of any other medical illness in past. • No history of any drug or substance abuse .
- 6. Past surgical history History of ankle fracture in childhood . No other history of any surgerical procedure or hospital stay for any illness.
- 7. Family history • No family history of psychiatric illnesses in first degree relatives . • He has 6 sblings and he is 4th among sblingship. • Father is 65 years old alive and mother is dead 5 years ago.
- 8. Pre morbid personality : • Before illness my patient was described by following attributes • Calm but short tempered • Dedicated toward his Job and work assigned to him • Like to enjoy his own company rather then being,social. • Over thinker.

Personal history: • Born with SVD at home ,no history of prenatal and antenatal complications, he achieved his milestones at average age .started going to school at age of 5 years , was average in studies • Achieved puberty at the age of 17 years ,and already knew about it from the counseling by his cousin . • Hetrosexual orientation • Married at the age of 28 years with his cousin sister, satisfied married life • Have 5 children from his wife and wants his elder most dughter to become an almma and teach Quran .

MSE ( mental state examination) • Appearance and behaviour : a 49 yrs old ,male of an average height and built ,,not maintaining eye contact ,sitting straight on the chair . • Speech: irrelevant • Mood and effect : irritatble mood with suspicious effects on face. • Sensorium / orientation: well oriented • Perception: altered perception • Thoughts : delusional thoughts followed by hallucinations.
