PROJECT DESCRIPTION LICENSED SITE PROFESSIONAL SERVICES Wastewater Treatment Plant OHI Engineering provided Licensed Site Professional Services for a release of 4,000 gallons of Ferric Chloride at a wastewater treatment facility in southeastern Massachusetts. Ferric chloride is a common compound used in wastewater treatment for pH adjustment. The release impacted soil, surface water, a detention basin and nearby wetland. OHI coordinated with the appropriate regulatory agencies and the emergency response contractor to complete remediation work and to restore the property to pre- release conditions. PROJECT DESCRIPTION LICENSED SITE PROFESSIONAL SERVICES Industrial Facility – Southeastern Massachusetts OHI Engineering is providing Licensed Site Professional services for an industrial facility in southeastern Massachusetts. The facility manufactures metallic plates for photoengraving purposes. As part of the manufacturing process, mineral spirits and other petroleum products are used for lubrication. A layer of these petroleum products has formed on the groundwater table due to leaks in the oil recovery system and from underground storage tanks. An extensive site assessment was conducted in the interior of the building. Issues overcome included small workspace, ensuring that manufacturing operations were not negatively affected, the significant thickness of the existing concrete slab and the fairly large extent of impact. In some wells, over three feet of non-aqueous phase liquid (NAPL) were identified. Free-product recovery is on-going. Periodic monitoring has indicated that NAPL layer thickness has been significantly reduced since the start of product recovery. OHI Engineering prepared submittals to the Massachusetts Department of Environmental Protection that modified the groundwater classification at the site so that drinking water standards no longer have to be met resulting in a significant cost and time savings.

PROJECT DESCRIPTION LICENSED SITE PROFESSIONAL SERVICES Residential Development – central Massachusetts OHI Engineering provided Licensed Site Professional Services for a property currently being developed as a residential subdivision. As is common near the hometown of Johnny Appleseed, this large property had been used as an apple orchard for approximately 100 years. Orchard operations included the application of pesticides including lead arsenate (pre-WW II) and DDT. Residual pesticides remained on-site at concentrations above the Massachusetts residential standard. An additional area of the farm included a “pit”, where concentrations of oil and leachable lead exceeded residential standards. OHI’s response action to address the oil and leachable lead at the “pit” included in-situ soil stabilization to reduce leachable lead concentrations to levels below those considered hazardous. The treated soil was excavated and hauled off-site for disposal. The appropriate reports and submittals were made to the Massachusetts Department of Environmental Protection. The area currently contains three newly constructed homes. The former pesticide mixing area had extremely high concentrations of organochlorine pesticides in soil. The soil was excavated and disposed of at a special facility in Quebec, Canada. PROJECT DESCRIPTION CONSTRUCTION SERVICES Dumping Ground Remediation – southeastern MA OHI Engineering provided professional services to the site’s General Contractor during remediation of an unpermitted dumping ground located on State property. The unpermitted dumping ground contained approximately 35,000 cubic yards of solid waste including metals, trees and stumps, plastic, tires, glass and other debris, and soil. The material was excavated and screened. Soil was reused where appropriate and several waste streams were created for off-site disposal. . OHI prepared a Health and Safety Plan (HASP) and Stormwater Pollution Prevention Plan (SWPPP). OHI’s on- site personnel conducted soil screening, dust monitoring and construction observation services during excavation activities, coordinated stockpiling procedures, collected samples for laboratory analysis of the stockpile and at the limits of excvation, and identified appropriate disposal facilities. PROJECT DESCRIPTION LICENSED SITE PROFESSIONAL and CONSTRUCTION SERVICES New Police Facility – Boston, MA OHI Engineering provided professional services to the site’s General Contractor during construction of a new Police building in Boston, MA. The construction site previously contained a metal plating operation that contaminated the property with metals and chlorinated solvents. Site work was complicated due to the limited storage space available on the property for the soil stockpiles and due to winter weather conditions. OHI provided soil screening and construction observation services during excavation activities, coordinated stockpiling procedures, collected stockpile samples for laboratory analysis, and identified appropriate disposal facilities for the soil stockpiles. - 12. PROJECT DESCRIPTION UNDERGROUND STORAGE TANK CLOSURES OHI Engineering, Inc. has completed underground storage tank (UST) at numerous residential, industrial and commercial properties. Facilities have ranged from large multi-story factories to small commercial facilities and single family residences. OHI has prepared and obtained permits from local conservation commissions for work near wetland areas. In several cases, additional evaluation involving sampling and analysis of soil and/or groundwater samples as well as other regulatory compliance activities were required to resolve releases from the USTs. UST closures typically involve observation during excavation and removal of the tank, field screening of soil remaining after tank removal and sampling and laboratory analysis of soil and/or groundwater samples. In some cases, releases of petroleum or hazardous wastes are identified that require compliance with appropriate state regulations. OHI can guide you through the regulatory process in a cost effective and expeditious manner. In one case, an unknown UST was in a timely manner without significantly encountered during construction of a affecting the construction schedule. new residence on a lakeside property. PROJECT DESCRIPTION – Remediation LICENSED SITE PROFESSIONAL SERVICES Silver Lake Regional High School During construction of an addition to the existing High School, soil and groundwater impacts associated with a former gasoline-containing underground storage tank were identified. The impacts caused construction issues relative to the installation of a portion of the stormwater control system. Due to the conditions encountered, notification of the Massachusetts Department of Environmental Protection was required and subsequent activities have been conducted as an Immediate Response Action (IRA) in accordance with the Massachusetts Contingency Plan. OHI Engineering developed the necessary health and safety procedures to allow installation of the stormwater system to proceed thereby avoiding delay to the construction schedule. Assessment activities were conducted to determine the extent of gasoline impacts to the soil and groundwater at the project site. A pilot test was conducted for design of a remediation system. High vacuum extraction was the chosen technology due to the presence of both soil and groundwater impacts in a relatively low permeability soil strata. PROJECT DESCRIPTION LICENSED SITE PROFESSIONAL and CONSTRUCTION RELATED SERVICES New Police and Fire Facility – Revere, MA OHI Engineering provided professional services during construction of new Police Headquarters and Fire Station buildings in Revere MA. The construction site previously was a dumping ground that contained numerous strata of various fill materials containing elevated levels of metals and petroleum compounds. OHI provided soil screening and construction observation services during excavation activities, coordinated stockpiling procedures, collected stockpiles for laboratory analysis, and identified appropriate disposal facilities for the soil stockpiles. Approximately 4,000 cubic yards of soil were disposed of off-site. OHI provided Licensed Site Professional services and completed appropriate reports and submittals to the Massachusetts Department of Environmental Protection. - 15. PROJECT DESCRIPTION LICENSED SITE PROFESSIONAL SERVICES John D. McGovern Middle School Medway, Massachusetts The John D. McGovern Middle School in Medway, Massachusetts, contracted the services of OHI Engineering Inc. to oversee the removal of a historic underground storage tank located behind the school. During the removal of the tank, petroleum impacts to the soil where noted. Due to the conditions encountered, notification of the Massachusetts Department of Environmental Protection was required and subsequent activities where conducted as an Immediate Response Action in accordance with the Massachusetts Contingency Plan. OHI oversaw the removal of the tank, disposal of the tank and contaminated soil, and subsequent closure operations while taking into consideration the special concerns of the Site as a functioning school. OHI developed a comprehensive plan to ensure that a condition of no significant risk was present at the Site. Assessment activities were conducted to determine the extent of the petroleum impacts to soil, groundwater and air. Soil borings and groundwater monitoring wells did not detect further petroleum impacts. To ensure the safety and health of the occupants of the Site OHI conducted a soil-gas survey by installing probes adjacent to the building in the vicinity of the release. The assessments are typically performed as a component of a property transaction. In several cases, additional evaluation involving sampling and analysis of soil and/or groundwater samples were required to resolve potential recognized environmental conditions. Environmental Site Assessments typically involve a review of historical information, town files, state files, and a site visit. Potential recognized environmental conditions are identified and resolved through further evaluation. Subsurface investigations may be warranted to resolve potential recognized environmental conditions. PROJECT DESCRIPTION HYDROGEOLOGICAL EVALUATIONS OHI Engineering provides professional services for hydrogeologic evaluations for wastewater treatment facilities and subsurface absorption systems, stormwater management and for contaminant plume migration studies. The evaluations typically include installation of groundwater monitoring wells, field tests to determine soil permeability and conductivity, and computer mounding simulations to determine future groundwater conditions. A Hydrogeological Evaluation was conducted for a 24,200 gallon per day soil absorption system associated with a private wastewater treatment facility for a residential development. Projjectt Experiience Pro ec Exper ence Mr. Borrebach was the Licensed Site Professional of Record for a release of approximately 600 gallons of PCB-containing transformer oil that occurred during decommissioning of an industrial site. The oil was released in close proximity to a major river in Massachusetts. Shortly after release, a large sheen was noted on the river, and immediate response actions were undertaken to place booms in the river to contain the sheen. The released oil moved onto a nearby beach and impacted the beach and the beach grasses. The release also affected soil in the vicinity of the vault system, sheet piling and piles along the riverfront. Another significant concern was the presence of shell fishing beds further downstream from the release. Assessment and remediation of the release included soil excavaction, cleaning and removal of the vault system, and remediation of the affected beachfront. A Site Specific Risk Assessment was prepared as impacts extended to surface water and sediments. The Risk Assessment showed that the remediation was successful in achieving a condition of no significant risk to human health, welfare, safety and the environment. Closure reports were prepared and submitted to the EPA and to the Massachusetts DEP. Work at the site was regulated by both the USEPA under TOSCA and the Massachusetts Department of Environmental Protection. He served as project Licensed Site Professional for several construction projects including the Teddy Ebersol fields in Boston, MA, the Boston Police Department facility in Roxbury, MA, runway improvements at Hanscom Field, the New Everett Park in Everett, MA, and closure of an unpermitted dumping ground in southeastern Massachusetts. These projects typically included soil screening, soil sampling and laboratory analysis, soil disposal facility identification and preparation of soil disposal paperwork, dust monitoring, health and safety plan preparation, preparation of Remediation General Permits and Stormwater Pollution Prevention Plans as well as submittals made to the Massachusetts Department of Environmental Protection to maintain compliance with the Massachusetts Contingency Plan. Mr. Borrebach was the Licensed Site Professional of record for a release of over two hundred gallons of residential heating oil at an ocean front residence in Osterville, Massachusetts. Soils, both outside and underneath the residence were impacted by the release. Mr. Borrebach was responsible for developing a structural shoring system to support the three story residence while excavation was conducted underneath the foundation of the residence. An area of the foundation of over thirty linear feet was exposed during the excavation work with no impact to the stability of the structure. Localized groundwater extraction and treatment was also conducted. Remaining oil concentrations were reduced to the point where a Class A-2 Response Action Outcome was achieved. Mr. Borrebach has been involved in providing environmental reviews, assessments, remediation feasibility studies and closure reports for Underground Storage Tank removal projects at gasoline stations, industrial facilities, manufacturing companies and residences. His responsibilities included verifying soil and groundwater conditions through field screening techniques, developing and implementing remedial measures, and submittal of closure reports while ensuring compliance with local, state and federal regulations, and in coordination with owners, permitting agencies and contractors. He was responsible for the assessment of an industrial property where high concentrations of chlorinated solvents were discovered. He designed and implemented a remedial system pilot test and installed a dual phase extraction system for the simultaneous removal of soil vapor and groundwater. The design resulted in a groundwater extraction rate that was increased by a factor of ten over that achieved by a typical groundwater extraction well. The design included retrofitting existing monitoring wells to serve as extraction wells thereby resulting in a significant cost savings for system installation. Mr. Borrebach prepared the Release Abatement Measure 2
- 20. Plan (RAM Plan), Phase 1 Initial Site Investigation Report, and Tier Classification submittals for the Site in compliance with the Massachusetts Contingency Plan. Groundwater analyses indicated a reduction in dissolved Chlorinated solvents of over ninety (90) percent after the first three months of system operation. He served as the Project Manager for site engineering design for a 172-unit housing development in Haverhill, Massachusetts. He was responsible for stormwater control system design, which included subsurface detention structures and improvements to the City of Haverhill combined sewer system to eliminate stormwater flow from the project area to the wastewater treatment plant. He was also responsible for site grading, sewer system design, design of the water supply system including off-site improvements, preparation of an Environmental Notification Form submitted under Massachusetts Environmental Policy Act (MEPA), and for preparation of a Stormwater Pollution Prevention Plan under a National Pollution Discharge Elimination System (NPDES) General Permit for Construction Activities in Massachusetts. Representative Project Experience Major Petroleum Bulk Storage and Energy Facilities Mr. Morrison served as the Project Geologist responsible for the coordination and management of project operations for major oil company retail facilities and active bulk terminals in eastern Massachusetts and Rhode Island. Responsibilities have included design of monitoring well placement and construction; well log and soil interpretation, water quality sampling and monitoring; aquifer evaluation; data control and graphics preparation for detailed waste site disposal and hydrogeologic investigation reports. Projjectt Experiience Pro ec Exper ence Mr. Snow served as the Senior Project Manager and LSP for a project on the north shore (Massachusetts) where petroleum vapors were detected in a daycare adjacent to the client’s property. The project was also within the Zone A of a Class A surface water body. The project included the restarting and redesigning of an existing air sparge and soil vapor extraction system. During the other firm’s initial indoor air assessment and risk assessments, an Imminent Hazard (IH) was identified. At the client’s request, I reviewed indoor air assessment and risk assessment and revised the conclusions to more accurately evaluate risk posed by the client’s release. Subsequent investigations of vapor intrustion pathways included the installation of soil gas probes under and around the daycare at various depths, and sampling of indoor air, soil gas, groundwater, and soil. Numerous IH and substantial hazard evaluations were also completed using Method 3 Risk Characterization. A sulfur hexafluoride tracer gas study was conducted in order to determine the migration pathway of contaminants to the daycare. Contaminants not related to the disposal site were ruled out and attributed to the other remaining businesses in the daycare building. I prepared a Immediate Response Action (IRA) Plan, IRA Status Reports, an IRA Completion report, Phase I, II, III, IV, and V documents including a Remedy Operating Status Opinion for the site. I also designed the Phase II investigation, a pilot test, additional remedial components and demonstrated vacuum influence and source control. I also provided support for litigation related to the indoor air impacts and to previous parties contributing to the release(s). Mr. Snow was the Senior Project Manager and LSP for a former industrial site in western Massachusetts. The site was impacted with PCBs, Metals and petroleum products. A previous consulting firm conducted an ASTM Phase I for the owner prior to the purchase stating that the parcel was not likely impacted. Subsequently a second ASTM Phase I was conducted by a potential purchaser, leading to an ASTM Phase II Investigation. The property was previously utilized by a major electrical utility company, a junk yard, and machine shop. The property was also adjacent to multiple rail lines and had a former rail spur and siding through the property. He designed an MCP Phase II investigation to delineate the PCBs and metals. Since the project was not in compliance with MCP deadlines, revised deadlines were negotiated with the MassDEP and penalties were avoided. Impacts at the site extended beyond the property borders, requiring access to multiple parcels including property owned by the power company and the city. The project also included legal support with regard to potential suits against the power company (source of the PCBs) and the first consultant. The project included multiple meetings with counsel, opposing counsel, and the MassDEP to gain access and determine damages. Multpile cost estimates and scenarios were presented to counsel and the client regarding potential remedial and long term management costs. Draft AULs were prepared. TOSCA did not apply to the site and the PCBs were averaged to below S-1 Standards. Mr. Snow served as the Senior Project Manager and LSP for an emergency response in Lowell, Massachusetts. An owner of a restaurant complained to the Fire Department about petroleum vapors in the basement of their building. Mr. Snow met the Fire Department, Police Department, MassDEP, and Water and Sewer Department at the site. He determined the source of the release was a nearby gasoline station. A carbon filtration system was installed in the basement of the restaurant as well as a series of monitoring wells and soil gas probes. A soil vapor extraction system (SVES) pilot test was completed and a temporary SVES was installed to provide vacuum control and abate an IH condition created by an explosive condition in the sewer lines. The temporary SVES was operational on the second day after the complaint. Follow-up work included the preparation of MCP deliverables, correspondence with the MassDEP and Fire Department as well as the permanent design and install of the SVES and migration pathway assessment. 2

He prepared contractor bid documents for solicitation of bids for remediation. Mr. Daley performed SVE, and groundwater extraction pilot testing, and prepared pilot testing report and developed remediation plans. Mr. Daley managed the comprehensive site investigation and remediation of a perchloroethylene/trichloroethylene release site. Contamination of nearby public water supply wells and complex geologic setting required special assessment methodologies, and QA/QC procedures, which were conducted with a high level of regulatory oversight, and public involvement.
