cardiorespiratory distress. He is alert and oriented to time, place, and person. The

ambulates to the examination room without assistance. He is able to sit comfortably on

examination table without difficulty or evidence of pain.
Respiratory: Lungs are clear to auscultation and percussion. No wheezes, rales, rubs
or rhonchi
are noted.
Cardiovascular:
On inspection there is an unilateral bulging in the left upper chest, there is an unilateral
retraction in the right lower chest, there is a localized bulging in the right lower chest,
there is a
localized retraction in the right upper chest, there is spinal deformity(scoliosis ).
Gastrointestinal: The abdomen is soft, non-tender without rebound or guarding.
Normoactive
bowel sounds are heard without rush. No palpable masses or hepatosplenomegaly is
appreciated.
No visible scars, hernias or sinuses are seen.
Neurologic: Alert and oriented X 3. Cranial nerves II-XII are grossly intact. Strength
5/5 in all
muscle groups. Sensation is intact to light touch and pinprick. Reflexes are equal and
symmetric
bilaterally in the upper and lower extremities. Babinski is negative. Cerebellar function
grossly
intact. Finger-to-nose coordination is within normal limits. Gait normal without ataxia.
Psychiatric: Patient is conscious, cooperative and well oriented to time, place and
person. There
are no mood swings or psychotic features. Patient's insight is good. Memory and
judgment are
intact.
