Form 1095-C is filed by Applicable Large Employers (ALEs) to report employee
health coverage under Section 6056. It must be submitted with Form 1094-C
to the IRS, and copies of Form 1095-C must be provided to employees.
3. Who Must File Form 1095-C?
Applicable Large Employers (ALEs) must file Form 1095-C if they:
● Have 50 or more full-time employees (or full-time equivalents)
● Offer health coverage to full-time staff
Each full-time employee must receive a copy of Form 1095-C.

● Employees’ name, SSN, and address
● Employer’s name, EIN, and address
