UNSAFE ACT OR CONDITION AROUND US
Material handling without proper PPE. While taking
heavy material without proper PPE is an laxness by
employee. As we know 3000 unsafe act and condition
cause 1 fatality, so we need to wear proper PPE while
working.
Fire Extinguisher is not placed at the proper
places. In case of any emergency we can’t use
properly which cause massive effect in
environment.
So always fire extinguisher kept in every entry and
exit position. In high risk zone kept in every 15m.
6. 6
OTHER EXAMPLE OF UNSAFE ACT OR CONDITION
The Earth Pit inspection due date has expired. Material are scattered here and there
Poor maintenance of electrical panels

CONCLUSION
If we stop the creation of 30,000 Unsafe Acts and
Unsafe Conditions then 3,000 Near Misses, 300
Minor, 30 Major Accidents will be stop and most
importantly 1 Fatal can be stopped.
