National Health and Nutrition
Examination Survey
• Assess the health of the US population each year
• N = 5000 / year
• 15 PSUs
• Nationally representative
• All ages
• HH questionnaire
• Extensive physical examination, specimen
collection, and follow-up exams / questionnaires

Topics
5/21/2012 4
Oral Glucose Tolerance
• Goal: to reassess the prevalence of diabetes and
impaired glucose tolerance in the US population
ages 12 and older.
• Protocol
– Fasting blood glucose test
– Drink sweet liquid
– Blood glucose test
• Problem: reminder letters and phone calls not
consistently reinforcing fasting instructions
(compliance).

Sexually Transmitted Disease (STD)
Results
• Goal: to ensure that respondents call in for their STD
results. Eligible respondents are 14-59 years old.
• Protocol
– Specimen collection at MEC
– Respondent provides password at MEC to receive results
– Laboratory processing for chlamydia, genital herpes, HIV,
female human papillomavirus
– Respondent calls NCHS physician and provides password
for results
• Problem: decline in proportion of respondents who
call-in for results (compliance)

STD Results Compliance
Text Messaging Objective
Contact respondents with text message
reminders to increase OGTT fasting
compliance and STD call-in compliance.

Obtaining Permission to Text
• Permission requested during home interview
• Age
– 12-15 year olds: parents respond
– 16+: answer for themselves
• Describe messages to be received
• Obtain respondent cell phone number

Limitations
• Not an experimental design
• Text messaging added to existing protocol
• STD call-ins not parsed by age group and
differing protocol
• Some text messages not received for unknown
reasons
5/21/2012 16
Conclusion
• Text messaging is easy to implement and cost effective
• OGTT
– Cell-phone users have greater compliance than non-cell
phone users
– Compliance with text messaging higher than 1999-2009
• STD
– Cell-phone users have greater compliance than non-cell
phone users
– Compliance improved with text messaging, but not as high
as 1999-2003.
• Plans underway to include text messaging for other
parts of NHANES
