Survey Checklist
Once you’vedrafted yoursurvey,revieweach survey question againstthechecklistbelow to be sure
you’refollowing bestpractices.
 Do youhave an “introductorystatement”withall piecesthatseemappropriate (name of
organization, relationshiptoparticipant,goalsandobjectivesof survey,significance of resultsto
respondent,basisof inclusioninsampleselection,participationisvalued,there are nocorrect
or incorrectanswers,all answersare anonymous,if applicable,name andpositionof
interviewer)?
 Are the firstquestions onyoursurvey easy/factual?
 Are sensitive questionsnearthe end of the survey?
 Are reflexive responsequestionsminimized,oristhe respondentsensitized (have you
emphasizedsubtle differencesbyunderliningoritalicizing)?
 Have you clarifiedmultiple v.singlechoice responses?
 Are you usingclosed- v.open-endedresponsesappropriately?Doyouhave "other"open-ended
responseswhere needed?
 Have you done awaywithall acronyms/abbreviations?
 Have you avoidedjargon?
 Are there any ambiguouslywordedquestions?
 Couldwordingbe simplified,isitconfusing?
 Are there any double-barreledquestions?
 Have you accidentallyincluded manipulative language? If youwanttoknow the outcome of
usingmanipulative language,have youfirstaskedastraightforwardquestionfollowedbyone
withmanipulative information?
 Are response optionsorderedinalogical way?If there isno logical way,are theyalphabetized
or randomized,whichevermakesthe mostsense?
 Are interval scale categoriesequalinsize (if itmakessenseforthemtobe so),do theyoverlap,
have youprovidedanunboundedfinal category?
 Have you providedappropriateresponsechoiceswhere needed,suchas“N/A”or “Unsure”?
