Vbs Registration 09

This document provides information about registering children for Vacation Bible School at Our Saviour Lutheran Church from June 29th to July 2nd, 2009. There is a $10 per child or $20 per family registration fee. Registration forms are due by May 24th. The form requests names, grades, birthdates, allergies, and emergency contacts for up to four children per family. It also asks for parent contact information and which areas they can assist with like planning, decorating, or teaching.
