Relatively cheap imaging technique extremely useful for the diagnosis of
abdominal patology
Useful aslo for the transabdominal evaluation of the gastrointestinal
tract
• The transabdominal gastrointestinal
ultrasound allows the evaluation of
the entire digestive tract
• oesophagus (especially the oesogastric
junction)
• stomach
• duodenum
• small bowel
• appendix and colon.
• Transabdominal gastrointestinal ultrasound (GIUS) - non-invasive
evaluation of the bowel including extra-intestinal features such as the
splanchnic vessels, mesentery, omentum and lymph nodes.
• Good accuracy and reproducibility not only in a primary work up, but
also in the follow up of chronic diseases.
• extensive data for the usefulness of GIUS in clinical practice
• fully implemented only in some countries
• lack of standardization of the examination technique, and of
guidelines.

• The echographic evaluation of the digestive tract is
addressed to experienced sonographists who are able to
recognize all abdominal and parenchymatous structures.
• Several additional techniques
• progressive compression of an air-containing organ,
• water administration in order to improve the stomach visualization,
or
• hydrosonography
• the use of 3.5 MHz convex and 5-7.5-10 MHz linear probes, and the
use of Power Doppler
• may help to better visualize the digestive structures.

• The ingestion of adequate amounts of water improve the assessment of
the stomach wall.
• The distention of the colonic lumen with instillation of water into the
colon, hydrocolonic ultrasound and with oral administration of
hyperosmotic solutions allows the visualisation of the colon.
• For the small bowel - ingestion of small (250 – 500 ml) amounts of iso-
osmolar polyethylene glycol (PEG) solution - US-enterography or Small
Intestine Contrast US (SICUS).

• The sonographic aspect of the GI tract normal wall is represented by 5
layers:
• a hyperechoic inner layer – commonly this represents the border
between the digestive fluid and the mucosa.
• a hypoechoic layer – commonly thin, represents the mucosa, lamina
propria, lamina muscularis.
• a hyperechoic layer – the submucosa.
• a hypoechoic layer – the muscular layer; its thickness depends upon
the segment of the digestive tract.
• an outer hyperechoic layer – the serous layer, the border to the
peridigestive fat.
